If you have not yet registered, please register first to receive an EduVision Student ID (e.g., S12345), which will be needed to complete this form. This can be found next to your child's name in the "My Child" page.
Please note that all students are entitled to one trial lesson per course. This lesson is limited to in-class teaching, and does not include homework submission and feedback. If your child subsequently enrols in the course, the trial lesson will be included in the course fee.
Trial lessons are scheduled by our office staff. One of our consultants will be in touch with you shortly after submitting this form.
